Trends in Peptic Ulcer Surgery


Abstract
To evaluate the effect of H2 receptor antagonists and fiberoptic endoscopy on the surgical treatment of peptic ulcer diseases, we reviewed 103 cases of peptic ulcer surgery, done at the department of surgery, National Police Hospital, during the
period
of 10 years from 1981 through 1990, and 48 articles published in the Journal of Korean Surgical Society during the period of 30 years from 1961 through 1990.
1) The number of gastric ulcer operation has decreased whereas that of duodenal ulcer operation has increased.
2) The most prevalent age group in duodenal ulcer was twenties(younger than before), and that in gastric ulcer was forties and fifties(older than before).
3) The number of acute ulcer patients increasing.
4) Perforation is the most common indication for operation.
5) The incidence of gastric ulcer bleeding and duodenal ulcer perforation is increasing, whereas that of duodenal ulcer obstruction and gastric ulcer perforation is decreasing.
6) The number of emergency operation is increasing whereas that of elective operation decreasing.
7) The most commonly used procedure for duodenal ulcer was vagotomy, and that for gastric ulcer was subtotal gastrectomy.
8) The incidence of duodenal ulcer has not been changed yet.
